The Personal Care Attendant Program is a MassHealth-funded service which helps people with permanent disabilities live at home by providing the funds to hire Personal Care Attendants (or PCA’s).  PCA’s give physical assistance with the “Activities of Daily Living” (ADL) that you cannot do yourself.

Step 1:

Speak with your primary care physician. If she/he believes you require assistance with Activities of Daily Living, request a Letter of Medical Necessity confirming your medical history and stating your need for services.

Step 2:

Complete the provided Intake Form.

Step 3:

Send a completed intake form and supporting medical documentation by fax to: 617-744-1427.

Once your Intake Form and Letter of Medical Necessity are received, a representative from UCP of MetroBoston’s Personal Care Attendant Program will contact you to schedule an in-person intake and orientation home visit.
